Understanding it

Understanding carpal tunnel syndrome

Symptoms
  • Tingling or numbness in the thumb, index and middle fingers
  • Symptoms that wake you at night, eased by shaking the hand
  • Clumsiness with buttons, coins and small objects
  • Aching in the wrist and forearm
  • Weakness of grip or pinch
  • Symptoms building during driving or phone use
What causes it
  • Compression of the median nerve as it passes through the wrist
  • Swelling in the tunnel from repetitive hand use or sustained positions
  • More common in pregnancy, with thyroid conditions and with diabetes
  • Sometimes no clear cause: the tunnel is simply snug
When to seek help
  • If tingling or numbness is waking you at night
  • If symptoms have lasted more than a few weeks
  • If grip is weakening or muscles at the thumb base look smaller
  • If constant numbness has replaced the tingling, seek assessment promptly
How we treat it

Our approach to carpal tunnel

Confirm the diagnosis, calm the nerve, and be honest about when surgery is the better route.

Your assessment confirms whether the median nerve at the wrist is truly the culprit, or whether the neck or other sites are contributing, which changes everything about the plan.

Mild to moderate cases often respond to night splinting, nerve and tendon gliding exercises, and changes to the positions and tasks that provoke symptoms. Severe or progressive cases do better with a surgical opinion sooner rather than later, and we will tell you plainly if that is you.

“I had a full hip replacement following a cycle accident, and have daily physio visits on weekdays for three weeks. I am already able to walk short distances without a stick or crutch, and this is entirely due to the work they have done with me.”

Frequently asked questions

Why are my hands worse at night?
Wrists tend to curl during sleep, which narrows the tunnel and pressures the nerve. A night splint keeping the wrist neutral often improves sleep within days.
Can carpal tunnel get better without surgery?
Mild to moderate cases often do, with splinting, exercises and activity changes. Severe cases with constant numbness or weakness usually need a surgical opinion, and outcomes are better when that happens promptly.
How is it diagnosed?
Clinical tests are often enough. Where the picture is unclear or surgery is being considered, nerve conduction studies confirm it, and we can guide that referral with your consent.
Is it caused by typing?
Typing alone is not a proven cause, but sustained wrist positions and repetitive hand work can aggravate a snug tunnel. Set-up changes help either way.
What happens if I ignore it?
Long-standing compression can lead to permanent numbness and muscle wasting at the thumb. Persistent symptoms deserve assessment, not toleration.
