Understanding hip pain

  • Pain lying on the affected side at night
  • Stiffness putting on shoes and socks
  • Aching after walking or standing
  • Pain that travels down the thigh
  • A feeling of catching or clicking deep in the joint
Common causes
  • Tendon problems on the outside of the hip, common in women over 40
  • Osteoarthritis of the hip joint
  • Impingement and labral irritation in younger, active adults
  • Referred pain from the lower back, which assessment will rule in or out
When to seek help
  • If hip pain has lasted more than a couple of weeks
  • If night pain is disturbing your sleep
  • If stiffness is making shoes, socks and car seats difficult
  • If pain followed a fall, particularly in older adults
How we treat it

Our approach to hip pain

Accurate diagnosis first: outer-hip tendon pain, joint arthritis and back-referred pain all need different plans.

Your assessment separates the hip joint itself from the tendons around it and from the lower back, which refers pain to the hip more often than people expect. You leave knowing which is driving your pain.

Treatment combines hands-on work with progressive strengthening. For hip arthritis, the evidence is clear that the right exercise reduces pain and keeps you doing more, for longer. For tendon problems, load management makes the difference.

“I had a full hip replacement following a cycle accident, and have daily physio visits on weekdays for three weeks. I am already able to walk short distances without a stick or crutch, and this is entirely due to the work they have done with me.”

Frequently asked questions

Why does the outside of my hip hurt at night?
Pain lying on the side often comes from the tendons and bursa over the bony point of the hip. It responds well to specific strengthening and simple changes to sleeping position while it settles.
Is walking good for hip arthritis?
Yes, in the right doses, alongside strengthening. Your programme will find the level your hip tolerates and build it up gradually.
Could my hip pain actually be coming from my back?
Possibly. The lower back commonly refers pain to the buttock and hip. Your assessment tests both so treatment targets the real source.
Do you help with recovery after hip replacement?
Yes, in clinic or at home. Structured rehabilitation after a hip replacement helps you get the full benefit of the new joint.
How long does hip pain take to improve?
Tendon problems typically improve over six to twelve weeks of consistent rehabilitation. Arthritis is managed rather than cured, but most people can reduce pain and do more within a similar timeframe.
