Understanding it

Understanding shoulder pain

Symptoms
  • Pain reaching overhead, behind your back or across your body
  • Pain lying on the affected side at night
  • Weakness lifting or carrying
  • Gradual stiffening of the shoulder, losing range in every direction
  • Clicking or catching with certain movements
  • Pain that spreads down the upper arm
Common causes
  • Rotator cuff problems, from irritation through to tears
  • Frozen shoulder, where the capsule tightens and restricts movement
  • Irritation of the tendons or bursa with overhead activity
  • Referred pain from the neck, which the assessment will rule in or out
When to seek help
  • If shoulder pain has lasted more than a couple of weeks
  • If night pain is affecting your sleep
  • If the shoulder is stiffening and you are losing movement
  • If the shoulder was injured in a fall and pain or weakness is not settling
How we treat it

Our approach to shoulder pain

Shoulders need an accurate diagnosis first, because the wrong exercises make the wrong problem worse.

The shoulder is the most mobile joint in the body, and different problems need very different treatment. Your assessment works out whether the pain is coming from the tendons, the joint, the capsule or the neck, and you leave knowing which it is.

Treatment combines hands-on work with a progressive strengthening programme built for your specific diagnosis. Frozen shoulders follow their own timeline, and we will be honest about that while keeping you as comfortable and mobile as possible.

“I had a full hip replacement following a cycle accident, and have daily physio visits on weekdays for three weeks. I am already able to walk short distances without a stick or crutch, and this is entirely due to the work they have done with me.”

Frequently asked questions

How do I know if I have a frozen shoulder?
A frozen shoulder gradually loses movement in every direction, including when someone else moves it for you. Your physiotherapist can usually distinguish it from other shoulder problems in one assessment.
Why is my shoulder worse at night?
Night pain is common with rotator cuff and frozen shoulder problems, partly from lying on the irritated tissues. It usually improves as treatment progresses, and we can advise on positions in the meantime.
Do rotator cuff problems need surgery?
Most do not. Research supports progressive exercise rehabilitation as first-line treatment for most rotator cuff problems. If your assessment suggests a surgical opinion is worthwhile, we will say so.
How long does shoulder rehabilitation take?
Tendon problems improve over weeks to a few months with consistent rehabilitation. Frozen shoulders take longer and move through recognised phases. You will get an honest timescale for your diagnosis.
Can I still train while my shoulder recovers?
Usually yes, with modifications. Your programme will tell you what to keep, what to change and what to pause, so you lose as little fitness as possible.
